To sign up for Spring Sports, follow these steps:

 

n  Contact the varsity coach of your sport via email to indicate your interest.  If there is not a coach listed, please wait until we post that and then contact that coach.

n  Complete this Health History Form online if you have not done so already this year, either with a Fall Sport, or at the beginning of the school yearHealth History Form 

 

n  Submit your Physical Exam Information to the Main office if you have not done so already (Freshmen and first year students to Camden Hills only)

 

n  First day of Spring Sports is tentatively scheduled for March 29th pending approval from the State of Maine.

 

n  Some sports may not be competing interscholastically this winter.  We are still waiting for guidelines from the MPA.  What exactly that looks like is dependent on the sport and they all are a work in progress.

REQUIRED FORMS FOR PARTICIPATION IN ATHLETICS

Certain forms must be completed before an athlete can participate in sports team practices and games.

The Health History/Consent Form must be completed once each year. You may download the form below or pick up a copy in the main office.  Please note that this form includes acknowledgements that you have read and understood the Misuse of Chemical Substances policy and the Concussion Information sheet.

CHRHS requires a recent Physical Exam, given at 9th grade or later, on file before students may participate in a sport. A "recent" exam is defined as one completed during the calendar year of the 9th grade or later. Physicals are required by the first day of practice or a letter from their health care provider allowing participation. Your practitioner may use the form below to report on a student's physical exam, or they may use their own form. Practitioners may fax physical exam forms to 207-236-7813.

ACADEMIC ELIGIBILITY FOR CO/EXTRA CURRICULAR ACTIVITIES

Participation in extracurricular and co-curricular activities is a privilege and carries with it certain academic responsibilities. The following are the procedures for determining students’ extracurricular and co-curricular academic eligibility.

Grades will be checked at the end of each quarter and four weeks into each quarter. Any student failing one or more classes at grade check will be placed on academic probation for 2 calendar weeks. During that time the student can fully participate in all aspects of any sport, extracurricular and/or co-curricular activity. After 2 calendar weeks, the student must be passing all classes. If still failing, the student becomes ineligible until a time when he/she is passing all classes. For athletics, plays, and musicals, if a student becomes ineligible, he/she may practice with the team/cast, but will not be allowed to participate in any performances, scrimmages, exhibition, regular season, or post-season contests, nor will he/she be allowed to travel to away games with the team. For other activities, if a student becomes ineligible, he/she may not participate in meetings, trips, performances, etc. Incompletes are treated the same as failures. All other MPA minimum academic eligibilities will also apply.
